Breast Implant and Reshaping (Auto-Augmentation)
Implant removal and reshaping, also called auto-augmentation, is a procedure that removes breast implants while reshaping the remaining natural tissue to restore a balanced and natural breast contour. This technique is ideal for individuals who no longer wish to have implants but still desire a lifted, aesthetic result without the need for new implants.
What is the indication of autoaugmentation?
Over time, some patients choose to remove their breast implants due to capsular contracture, rupture, discomfort, or personal preference. While simple implant removal can leave the breast smaller or deflated, reshaping techniques use the patient’s own remaining breast tissue and skin to create a lifted, natural breast mound in the same procedure.
The operation may include:
-
Implant removal (usually without capsulectomy)
-
Internal tissue reshaping to restore fullness in the upper breast
-
Skin tightening or breast lift (mastopexy) to correct sagging
-
Repositioning of the nipple–areola complex for symmetry and proportion
This technique combines surgical precision with aesthetic principles to achieve a natural breast shape while maintaining safety and long-term balance.
When to Consider This Procedure
Implant removal and reshaping may be recommended if:
-
Implants are no longer desired or have reached their lifespan
-
There is capsular contracture or implant rupture
-
The breast appears droopy or deflated after implant removal
-
There are concerns about implant-related illness or discomfort
-
A more natural, maintenance-free result is preferred
The Procedure
-
Anaesthesia:
Performed under general anaesthetic for comfort and precision.
-
Implant Removal:
The existing implant is removed through the previous scar. If required, a total or partial capsulectomy is performed to remove the surrounding capsule.
-
Reshaping (Auto-Augmentation):
The remaining breast tissue is internally reshaped and lifted to restore upper-pole fullness and create a natural contour.
In some cases, the tissue previously stretched by the implant can be folded and anchored internally to mimic the shape of a small implant.
-
Nipple Repositioning:
The nipple–areola complex may be lifted to match the new breast shape and maintain natural proportion.
-
Closure:
The skin is tightened and closed with dissolvable sutures, and the incision is sealed with surgical glue or steri-strips.
-
Duration:
The operation usually takes 2–3 hours, depending on the amount of reshaping required. Most patients stay in hospital overnight.
After the Procedure
-
Mild swelling, bruising, or tightness is expected and settles within days.
-
You can shower from the day after surgery; dressings are waterproof.
-
Continue wearing your surgical bra day and night for at least two months to support healing.
-
Avoid strenuous upper-body activity for four to eight weeks.
-
Driving is permitted after your first postoperative review, once you are comfortable and off strong pain medication.
-
The final shape continues to improve as swelling subsides and tissue softens over several months.
Advantages and Disadvantages
Advantages:
-
Achieves a natural breast contour without new implants
-
Uses your own tissue for reshaping
-
Single procedure combining implant removal and lift
-
Quick recovery and durable results
Disadvantages:
-
Decrease in overall breast volume
-
Some scarring and skin tightening may be visible
-
Not suitable for patients with very little breast tissue
Possible Risks and Complications
As with any operation, potential complications include but are not limited to:
-
Bleeding or haematoma
-
Infection
-
Seroma (fluid collection)
-
Temporary numbness or change in nipple sensation
-
Asymmetry or contour irregularity
-
Delayed wound healing or minor skin or nipple loss
-
Fat necrosis (firm areas within the breast)
-
Need for revision surgery (rare)
-
General anaesthetic risks

Want implant removal without replacement
-
Have mild to moderate skin laxity or droop

Prefer a natural appearance with long-term stability
A detailed consultation, clinical assessment, and imaging are performed before surgery to design a personalised surgical plan.
